Appointments and Scheduling
Thank you for making an appointment at our office.
As a courtesy to all of our patients, we ask that you arrive within 5 minutes of your scheduled appointment; please call us if you expect any delay.
We reserve the right to reschedule patients who arrive more than 15 minutes after your scheduled appointment time.
Due to emergency situations, there may be a delay in your appointment. We will make every effort to contact you in advance of any such situation.
Payment Policies
Payment is expected at the time of service unless other policies apply.
Methods of payment include: Visa, MasterCard, American Express, Diners Club, Discover, personal check, certified check, and cashier’s check.
If you are interested, you may request patient financing information.
Consultations
The fee for an aesthetic consultation with Nilay Shah, M.D. is $75. Should you choose to schedule a procedure with the doctor, the fee is fully applied toward that procedure or service.
If you wish, please bring photographs of yourself or others that may help you to discuss your goals with us.
Office Visit Cancellation and Rescheduling
Please notify our office immediately if you must cancel or reschedule any office visit, consultation or in-office procedure.
Any cancellation of a consultation or in-office procedure less than 1 business days prior to your appointment may incur a cancellation fee.
Insurance Coverage
Insurance benefits do not cover procedures performed strictly to enhance your appearance.
The benefits paid by insurance companies for plastic surgery vary. Therefore, pre-approval of coverage and projected reimbursement is essential. Insurance coverage is your responsibility; however, we will make every effort to assist you in obtaining coverage where it is warranted.
You are ultimately responsible for the full payment of your account.
Deposit
A $500 deposit payable to Nilay Shah, M.D. will be required to reserve your surgery date.
Surgical Fees
All surgical fees and additional costs such as implants and garments are required paid-in-full to
Nilay Shah, M.D., 1 week prior to your surgery.
Other Facilities
All over-night (hospital) (recovery center) fees are required.
Rescheduling and Cancellation
If you must cancel or re-schedule your surgery within 48 hours prior to your procedure, you may incur a cancellation fee of $500.
Dr. Nilay Shah reserves the right to cancel or reschedule your surgery for your health or other reasons. All deposits and payments with exception of consultation fees will be refunded in the event of a cancellation determined by the doctor.
